Welcome to BridgesHome Health Care. Our mission is to provide health care that assists people to achieve the highest level of independence possible. We are the bridge from hospital, nursing home, rehabilitation unit, or long term care facility to home. BridgesHome Health Care is also a link between the patient, family, and the physician. We strive to carry out the physician’s plan for the patient with a full team of health care professionals that understand how the home can be a safe and healthful place.
BridgesHome is a not-for profit organization, started in 1997, that is a joint venture of Hospice of Medina County and Medina General Hospital. This means that BridgesHome offers the same caring, compassionate, and quality services provided by these two great organizations.
To be eligible for BridgesHome Health Care services, patients must have a physician referral prescribing the need for home health care. The referral may be initiated while the patient is hospitalized with an illness or during a stay in a rehabilitation unit or long-term care facility. Medicare guidelines require that the patient be homebound. The term "homebound" indicates that the patient has a condition that prevents leaving the place of residence on a regular basis. Patients may leave for medical appointments or for absences of infrequent or relatively short duration.
Skilled services include hands-on care and treatments such as wound care, injections and intravenous therapy, education on disease treatment and prevention, and assessments by registered nurses regarding the health and safety of the home. Rehabilitative therapy to restore strength and mobility and improve speech and function in tasks of daily living is provided by physical, occupational, and speech therapists. Social workers are also available to evaluate the social and emotional factors affecting the patient and family and identify available community resources. While a patient is receiving any or all of these skilled services, a home health aide may be provided to assist with personal care.
